AGREEMENT:
In addition to filling out this form, firm or corporation must submit a letter on its letterhead applying for credit voucher charge, and also indicate a list of names of authorized personnel who are allowed to place job orders. The above firm assumes all financial obligation and guarantees payment with regard to incurred charges on all vouchers for jobs ordered by authorized personnel. Clients listed here personally guarantee payment of any and all charges including but not limited to, all collection and attorneys fees in case of delinquency.

Corporate account cancellations can be made 4 days prior to the scheduled booking without incurring cancellation fees. No-shows will be charged a full price. In case of a conflict between the advertised price and/or the quoted price and/or the voucher price, the voucher price will prevail and the above firm is obligated to pay the full fare as stated on the voucher. It is the passengers responsibility to collect a copy of the voucher directly from the driver by the end of the ride.

All clients listed permit U-R-THERE-Limousine to submit unsigned credit card vouchers, stating that client's signature is on file, or to amend, alter, complete, or execute on behalf of the client, credit card vouchers in clients name, for payment of charges.

A U-R-THERE-Limousine representative will contact you for the credit card information once this application has been received and processed. Payment in full is due upon invoice receipt. If no payment is received upon receipt, the amount due will automatically be charged on the back-up credit card.
